I would say that the UK/Europe exclusive promos which come in the DVD's are worse. The card is literally just placed loose to rattle around the box. I must say though, I have had issue with the 4 indents on a lot of my tin promos in the past.

The reason this is different to the Dark Dragonite is because the Team Rocket set did contain a non-holo Dark Dragonite. Since the design process of the cards would include what the cards look like, rather than including details such as are the cards holo, this error almost certainly occured...
