Link to submit background check?

I sent in my background check paperwork about six weeks ago. However, four weeks ago I got this request from Pokemon.com:

As part of our ongoing efforts to have current background checks on file for all Premier Tournament Organizers, Tournament Organizers, and League Owners, you have been selected to take part in our current round of background checks.
Please go to background/instruction and follow the instructions found there in order to submit your background check paperwork.
If you have any questions about this matter, you may contact us via support.pokemon.com.

The text "background/instruction" appears to be a link, but it goes nowhere.

I would like to re-submit my info, but I cannot find any link to do so. I even tried submitting a 2nd application, but there was no "next step" with instructions like I got last time.

Does anyone have the link to the latest form?

Yeah..this is our situation..we get a dead link and we have written to the pokemon people repeatedly..so much that they send us messages that they are erasing our questions in the forums since they believe they are repeats... It's exasperating since we are trying to start a pokemon league here..

This is the same thing that happened to me. Now I can't find the link to print out the background check and mail it in again. Does anyone know where it is?
