If I have Relicanth SV active and use Amnesia attack on my opponent's active Machamp SF and specify that Machamp cannot use Take Out next turn... But, my opponent has a second Machamp SF on the bench and retreats the active Machamp, can my opponent use the Take Out attack of the new Machamp from the bench?

Yes. Amnesia only puts an effect on the Active Pokemon.
