My friend needs to send the "parental consent form" to Wizards in order to become a Professor. He does not have this document anymore, though. Has anyone got it? It's a 2 page long pdf file, if I remember right.

I believe this text from the ratings and rankings will answer your question:

In Europe and the Americas, we are requiring parental consent before including players under certain ages in publicly available city or regional rankings. If you are not appearing in these rankings, it may be because we do not have your birthdate on file or because you are in an age group for which we are now requiring parental consent. (Players under 13 years of age in the Americas, Eire, South Africa, and the U.K. must have parental consent before they can be included in city or regional rankings. Players under 18 years of age in continental Europe must have similar consent.) Please be sure to add your birthdate to DCI? records through the Personal Stats sidebar button. If you are in an age group requiring parental consent, you will be directed to a downloadable consent form after entering your birthdate in the appropriate field. If you have problems updating your information, please contact us at with your DCI number, name, date of birth, and postal/ZIP code.
