What cards are considered ban/restrictions from the tournament? Which sets are considered ban/restriction also? Is there any sites that I can look into it?

Well for DCI sanctioned tourneys, there are two (2) sets of restrictions, one each for the type of tourney:
Unlimited - Unlimited tournaments can use any card released by WotC except Ancient Mew and ________'s Pikachu aka Happy Birthday Pikachu (black star promo # 24).
Modified - for Modified tournaments, only certain sets and cards can be used. Currently any card from Neo: Genesis (except Sneasel) and higher can be used. So any of the NEO sets, Southern Islands, Expedition, and Legendary Collection can be used. Also if a card has been reprinted in a set, like the ones from Legendary Collection, you can use the original cards from the original sets the cards came in. Also Black Star promos numbered 21 and greater can be used, except # 24 ________'s Pikachu. And Ancient Mew cannot be used.

Now to stay on top of Modified, i'd click HERE! and then pick either the MS Word or PDF version of the Pokémon Floor Rules.
