Difference between deck formats?

I was wondering what each of the different deck formats mean..

I think this was how it goes:
Unlimited - All cards in the whole Pokemon TCG series are legal
Limited - All cards in the Diamond and Pearl Series along with the four most current Ex series
Modified Legal..? - I have no clue what this format includes..


Will you post the technical name of each deck format, what cards it includes, and what they are also known as?
