do i need to send a consent and release form for the top 3 in each age division or just 1st place in each age division?

also, i just noticed that the address for the event report form and the consent and release are the same, but the zip codes are different.

the consent and release form being 98102,
and the event report form being 98052.

which zip code should i send them to?

Consent Form

Hi. I believe the Consent and Release form is for the 1st place winner in each of the three age divisions. This is so POP can use their first name and last initial and/or a picture of the winners if you submit one.

As for the zip code, the correct one is 98052.
