Almost all previous DS-compatible Enigma Island cards are compatible with Diamond/Pearl using the same method as the GBA games. Remember to use DI for Diamond or PE for Pearl in the confirmation password. This post highlights the changes the islands have taken for Diamond/Pearl, including appearance and Pokémon. All islands have graphics enhanced for Diamond/Pearl. (Note that if it's said new varieties appear, unless it specifically states that its encounters replace that of an old variety, the old varieties can still be found. Additionally, Pokémon that have a tendency to only appear at certain times will only appear at those times unless noted otherwise.)
Abbreviations are as follows: SAE is Sonic Adventure Evolution. SH is Sonic Heroes. SPS2 is Sonic Promo Set 2. EIP04 is Enigma Island Promos 2004 Edition. EIP05 is Enigma Island Promos 2005 Edition. EIP06 is Enigma Island Promos 2006 Edition. JLM3 is Johto League Masters 3. S/C/EA is Shining/Crystal/EX Alliance. FF7 is the Final Fantasy VII set. DE is EX: Draco's Emergence.
Berry Secret (SAE/SPS2): The island is wider in Diamond/Pearl and has two berry-growing areas. The western one is eight 8-square-high columns and, when you first arrive, contains each of the berries found in the Diamond/Pearl in-game (there's 64 total). The eastern one is fifteen 10-square-high columns and one 9-square-high one (with the sign below the 9-square-high one) and, when you first arrive, contains each of the berries obtained through DS transmission that you've unlocked so far (the potential ceiling for this is 159, although there are doubts that this will be reached). There are no other changes.
"Ten" Island (EIP05/SPS2): No changes.
Sonic's Island (SAE/SPS2): The Pokémon variety has increased.
The e-Reader-transmitted version of the GBA island is the one whose music is used.
Tails's Island (SAE/SPS2): The Pokémon variety has increased. The sky zones on this island cover the entire complex that includes Enigma Island (that is, Sky Enigma, Sky Ludicolo, Sky Trainers', Sky Suicune, and Sky Scizor). Rayquaza may appear in the Diamond/Pearl sky zones, but you'll need to have an Emerald game pak in the GBA slot on your DS, and you can only find a Rayquaza in the sky zones while Tails's Island is the active Enigma Island. Additionally, you can only catch one Rayquaza in this location in Diamond/Pearl and Rayquaza is at level 70 (as opposed to 10 in the non-Emerald GBA sky zones).

Knuckles's Island (SAE/SPS2): The Pokémon variety has increased. Where you got a fossil on the GBA is now an entrance to the Sinooh Underground. There aren't any secret bases in the Sinooh Underground from here.

Shadow's Resort (SAE/SPS2): The Pokémon variety has increased.
The e-Reader-transmitted version of the GBA island is the one whose music is used.
Eggman's Resort (SAE/SPS2): Budew, Chingling, Bonsly, Mime Jr., Happiny, and Munchlax join the variety found in the grasses. You can now find Mantyke by surfing. All of those Pokémon can now be found in the eggs with the same moves as previously, except if you get a Mantyke from the right-hand egg, it will know Dragonbreath instead of Surf.

red face paint's Resort (SAE/SPS2): red face paint can now offer a Bronzong that knows Psycho Boost in the trade.

Chao Paradise (SAE/SPS2): The Chao transfer method is different in Diamond/Pearl. There are now two of them. The first is to insert up a Sonic Advance, Sonic Advance 2, or Sonic Pinball Party game pak in the GBA slot on the DS. You can transfer the Chao to that game's Tiny Chao Garden in that way. The other is to load up Sonic Adventure 2 Battle or Sonic Adventure DX: Director's Cut into a Wii, upload the password transmission program used for transmitting data to the Wii, and then make a transfer into a Chao Garden for the matching game on any Gamecube memory card inserted into the matching slots on the Wii.

Amy's Oasis (SH/SPS2): The hammer shack offers Signal Beam instead of Rock Slide in Diamond/Pearl.

Cream's Oasis (SH/SPS2): In Diamond/Pearl, the wild Mantine that were in the GBA games have devolved; that is, you now encounter Mantyke instead of Mantine. Starly can now appear in the grass. The computer can be used the same way the transporter on Chao Paradise is used in Diamond/Pearl.

Espio's Atoll (SH/SPS2): No changes.
Charmy's Atoll (SH/SPS2): No changes.

Trainer Island (JLM3): The cards used for battle transmissions are the ones used in the transmission of the Diamond/Pearl Battle Tower team. The trainers will use those teams in Diamond/Pearl. Watch for the teams in later sets.
Brock's Quarry (EIP04): A future set's piece of transmittable data will result in a rematch with Brock on this island. Once that happens, you'll find Brock at the rearmost point of the cave.
Surge Boot Camp (EIP04): All of the cadets have new teams in Diamond/Pearl. Some grasses will now appear if you have a FireRed game pak in the GBA slot on your DS, and these contain Elekid. A future set's piece of transmittable data will result in a rematch with Lt. Surge on this island. Once that happens, Lt. Surge will appear in the northwest corner of the camp once you've beaten all the cadets once. Here are the teams of the cadets:

Koga's Pagoda (EIP04): Some of the trades are different in Diamond/Pearl. The one who has Qwilfish now wants a Stunky (instead of Gulpin). The one who has the female Nidoran hasn't changed their trade, that one still wants a Roselia. The ones who have Nidorino and Weepinbell now want Skorupi (instead of Gloom) for the Nidorino and Croagunk (instead of Gloom) for the Weepinbell. The one who has Ariados also hasn't changed their trade, that one still wants a Golbat. A future set's piece of transmittable data will result in a rematch with Koga on this island. Once that happens, trade with everyone and Koga will appear by the front door.
Mt. Blaine (EIP04): You can no longer encounter Magby here unless you have a LeafGreen game pak in the GBA slot on your DS, but you can now find Vulpix and Torchic in the same area. A future set's piece of transmittable data will result in a rematch with Blaine on this island. Once that happens, catch Moltres. Once you've done that, find Blaine where Moltres once perched.
Giovanni's Lab (EIP04): You can choose between a Deepseascale and Deepseatooth when you come here for the first time. A future set's piece of transmittable data will result in a rematch with Giovanni on this island. Once that happens, Giovanni will be in the same place as he is normally, but will make you do a task to earn the rematch.
Falkner's Rest Stop (EIP04): You now encounter Mantyke instead of Mantine. The Emerald modifications are in place, including the sky zone here. A future set's piece of transmittable data will result in a rematch with Falkner on this island. He can be rematched the same way as you did in Emerald.
Bugsy's Paradise (EIP04): You must beat Bugsy in a rematch to access the trades, which are different. Bugsy offers Venomoth, but you must part with a Combee to get it--and he'll only take a female Combee. Misty now offers Clamperl, but you have to part with a Mantyke to get it. A future set's piece of transmittable data will result in a rematch with Bugsy on this island. He can be rematched the same way as you did in Emerald.
Morty's Haunt (EIP04): There are no changes from the Emerald version of the island. A future set's piece of transmittable data will result in a rematch with Morty on this island. He can be rematched the same way as you did in Emerald.
Chuck's Dojo (EIP04): The choosable Hitmontop holds a TMd01. A future set's piece of transmittable data will result in a rematch with Chuck on this island. He can be rematched the same way as you did in Emerald.
Pryce's Glacier (EIP04): Snover are now available in the snowbanks, as are Smoochum if you have Pokémon Mystery Dungeon: Red Rescue Team in the GBA slot on your DS. A future set's piece of transmittable data will result in a rematch with Pryce on this island. He can be rematched the same way as you did in Emerald.
Brawly Field (EIP04): Brawly has a different team for the rematch.
Wattson's Atoll (EIP04): Wattson has a different team for the rematch.
Norman's Beachfront (EIP04): Norman has a different team for the rematch.
Wallace Pier (EIP04): Wallace has a different team for the rematch.
Forest of Jason (EIP04): The Pokémon variety has increased. Jason has a different team for the rematch.
Danny's Chill Zone (EIP04): The Emerald modifications are in place, complete with the inside area. Snover are now available in the snowbanks in the inside area. Danny has a different team for the rematch.
Rudy's River (EIP04): The Emerald modifications involving Rudy are in place, but not those involving the Marine Cave. Rudy has a different team for the rematch.
Drake's Stadium (EIP04): Drake's Electabuzz is now an Electivire.
Bruno Arena (EIP04): No changes.
Will's Mind Sphere (EIP04): No changes.
The Lance Shrine (EIP04): There are now six additional spheres on the island, two of them in each of the rows. To activate the new ones, you need to have Gible, Gabite, Garchomp, Dialga, Palkia, and Giratina.
Sidney City (EIP04): The new mazes for the power-restoration mission are at the Valley Windworks in Diamond/Pearl. The second new maze has wild Electivire encounters instead of Electabuzz.
Drake Port (EIP04): The ship now beaches at Lake Verity. The pirates have new teams in Diamond/Pearl. Here's the rundown:

Shingo Boot Camp (EIP04): A future set's piece of transmittable data will result in a rematch with Shingo on this island. He can be rematched the same way as you did in Emerald.
Eusine Resort (EIP04): A future set's piece of transmittable data will result in a rematch with Eusine on this island. He can be rematched the same way as you did in Emerald.
Ritchie's Base (EIP04): A future set's piece of transmittable data will result in a rematch with Ritchie on this island. He can be rematched the same way as you did in Emerald.
Gary's Lab (EIP04): The 3:00, 10:00, and 11:00 hour encounters are different in Diamond/Pearl. You now see Budew at 3:00, Chingling at 10:00, and Happiny at 11:00.
Harrison's Hideaway (EIP04): A future set's piece of transmittable data will result in a rematch with Harrison on this island. He can be rematched the same way as you did in Emerald.
Juan's Secret Nook (EIP04): A future set's piece of transmittable data will result in a rematch with Juan on this island. Once that happens, Juan will appear in the northeast corner of the island, but will make you do a task to earn the rematch.
Tucker's Dome (EIP04): No changes.
Noland's Factory (EIP04): No changes.
Spenser's Palace (EIP04): No changes.
Brandon's Pyramid (EIP04): No changes.
Fighting Island (S/C/EA): Riolu can now be found in the grasses.
Fire Island (S/C/EA): Torchic can now be found in the Cave of Coal. Numel can now be found in the Cave of Kindling. Houndour can now be found in the grasses outside.
Grass Island (S/C/EA): You can now find Treecko inside the Tree of Foilage. Eevee now appear in the grasses on the base island.
Electric Island (S/C/EA): Electrike now appear inside the building. The Elekid encounter rate increases if there's a FireRed game pak in the GBA slot of the DS.
Psychic Island (S/C/EA): No changes.
Water Island (S/C/EA): A transmittable TM from a future set will allow you to use the Dive spot, which now has Mudkip underwater. (All islands that have Dive spots will allow said transmittable TM to be used there.)
Island of Darkness (S/C/EA): Mightyena now appear in the grasses. Carvanha can now be found by fishing.
Steelworks (S/C/EA): Aron and Mawile now appear inside.
Normal Island (S/C/EA): The Pokémon variety is changed. Instead of all-Kanto, the mix is all-non-Sinooh-Dex.
Oak's Remote Lab (S/C/EA): Instead of giving you the National Pokédex, Prof. Oak will simply check your Pokédex. If you have 99 species caught, you'll get a Nugget as a reward. 198 species caught nets you a TM26. 297 species caught nets you 5 Nuggets. 395 species caught nets you 5 TM26. Finally, all 493 species caught nets you 10,000,000 in money, split between your wallet and the Jackpot Bank. You can only collect for each milestone once.
Elm's Remote Lab (S/C/EA): Elm's variety of evolution items have increased.
Birch's Getaway (S/C/EA): The grasses now contain Zigzagoon and Linoone.
Misty Cape (EIP05): A future set's piece of transmittable data will result in a rematch with Misty on this island. Once that happens, you'll find Misty in the center of the island, but you'll need to use the itemfinding function to dig her out of the sand.
Erika Grassland (EIP05): Treecko and Seedot now appear in the grasses. A future set's piece of transmittable data will result in a rematch with Erika on this island. Once that happens, you'll find Erika in the center of the island, but you'll need to use Cut to get her out of the grass.
Sabrina's Floater (EIP05): Sabrina's trade is different. She still has a Starmie, but to get it, you'll have to give her a Bronzong. A future set's piece of transmittable data will result in a rematch with Sabrina on this island. Once that happens, she'll be in the same place she was before, but you'll have to have made the trade before you can battle.
Aeris's Plateau (EIP05): The password that transmits this island doesn't work for Diamond/Pearl, meaning that this island is, for the time being, incompatible with Diamond/Pearl. (There may be a way in a future set...)
The Janine Casino (EIP05): The prize table is different. The Emerald modifications are also in place. A future set's piece of transmittable data will result in a rematch with Janine on this island. Jasmine Bay (EIP05): If you have a FireRed game pak in the GBA slot in your DS, Elekid may appear in the wild inside the lighthouse. Aside from that, the Emerald modifications are in place, including Jasmine's appearance. A future set's piece of transmittable data will result in a rematch with Jasmine on this island. She can be rematched the same way as you did in Emerald.
Clair Falls (EIP05): The Emerald modifications are in place, including Clair's appearance. A future set's piece of transmittable data will result in a rematch with Clair on this island. She can be rematched the same way as you did in Emerald.
Mt. Roxanne (EIP05): Roxanne has a different team for the rematch. (The Emerald modifications are not in place.)

Tate & Liza's Skypad (EIP05): Various media reporters, be they TV, radio, newspaper, web site, etc., are now swarming the base island and each of the towers. The ones on the base island will remain there until you reunite Tate & Liza, while the ones in the towers will remain until the towers collapse. You don't have to fight any of the reporters. Tate & Liza have a different team for the rematch.
Cissy Downs (EIP05): The Emerald modifications are in place, complete with the rematch. The actual Surf race now takes place in the same perspective as watching the race in Emerald, and the controls are slightly changed; the GBA L and R are the DS Y and X, respectively. Cissy has a different team for the rematch. Once you've beaten Cissy once with a Sapphire game pak in the GBA slot on the DS, a tsunami will hit the island and the track will be evacuated. A cutscene depicting the track being damaged and the island being flooded will occur, and a dive spot will appear in front of where the door was. Once you use Dive, you'll find a cave entrance. This cave is the Marine Cave, and it works the same as it did in Emerald, right down to obtaining Kyogre. You must catch Kyogre in order to restore the island to normal (if you beat Kyogre without catching it, the Marine Cave won't vanish, the island will remain flooded, and you'll be able to rematch Kyogre on your next visit).
Hotel Luana (EIP05): The Emerald modifications are in place, complete with the rematch. This time around, however, there are now 20 floors to the hotel (there were 15 in the GBA games, something that was never quoted, especially since the 14th and 15th floors weren't important), your hotel room in Diamond/Pearl is 2006 instead of 1337, there are now 19 charms instead of 12, and there are a few slot machines on the ground floor. The new charms can be found in the following rooms: 1337, 1414, 1522, 1606, 1711, 1840, and 1928 (this room is the presidential suite). Luana has a different team for the rematch. Once you've beaten Luana once with a Ruby game pak in the GBA slot on the DS, an earthquake will hit the island and the hotel will be evacuated. A cutscene depicting the hotel being damaged will occur, and a cave will rise in front of the door. This cave is the Terra Cave, and it works the same as it did in Emerald, right down to obtaining Groudon. You must catch Groudon in order to restore the island to normal (if you beat Groudon without catching it, the Terra Cave won't vanish, the hotel will remain damaged, and you'll be able to rematch Groudon on your next visit).
Resort Lorelei (EIP05): Two of the trades are different. The one who has Cloyster now wants a Snover. The one who has Piloswine now wants Sneasel.
Agatha's Dusk Property (EIP05): A Dusknoir in your party has the same effect as Duskull or Dusclops. A Chingling in your party has the same effect as Chimecho. The Emerald alterations to the Duskull-line maze are in place.
Karen Village (EIP05): No changes.
Phoebe's Luau (EIP05): The method for obtaining the invitation is different. To obtain an invitation in Diamond/Pearl, either get one from a held Pokémon in Pal Park or, the first time you transmit this island into Diamond/Pearl, an Event Challenge will open in which you must beat Cal's team in one battle, a single battle with Arceus banned. Win that battle and you'll obtain the invitation. Here's Cal's team for the Event Battle:

Additionally, there are six new invitees in Diamond/Pearl. Fantina, Aaron, Bertha, Flint, Lucian, and Cynthia join the fun, bringing more rare in-game berries to the party. (For the record, yes, you can still participate in the Event Challenge even if you got an invitation through Pal Park.)
Glacia Glacier (EIP05): No changes.
Liza's Fire Maze (EIP05): Chimchar can now appear on the land inside the cave. A future set's piece of transmittable data will result in a rematch with Liza on this island. She can be rematched the same way as you did in Emerald.
Sonrisa Greenhouse (EIP05): Turtwig can now appear in the greenhouse. A future set's piece of transmittable data will result in a rematch with Sonrisa on this island. She can be rematched the same way as you did in Emerald.
Trixie's Space Stage (EIP05): You'll now find Trixie on Ludicolo Lane. The floor of the space station can now wield Piplup. The second warp tile leads to an otherwise-unreachable point in the Scizor Mountains. When Trixie enables the second warp tile, she'll also activate the teleporter that leads to the rest of the Space Station, which is now manned by only two astronauts on the roof in Diamond/Pearl (neither of which want to battle), and all the inside areas now can yield wild Pokémon (only the SE Wing did so in the third generation). A future set's piece of transmittable data will result in a rematch with Trixie on this island. She can be rematched the same way as you did in Emerald.
Electra Compound (EIP05): A future set's piece of transmittable data will result in a rematch with Electra on this island. She can be rematched the same way as you did in Emerald.
Milane's Weeds (EIP05): A future set's piece of transmittable data will result in a rematch with Milane on this island. She can be rematched the same way as you did in Emerald.
Luka Sea (EIP05): A future set's piece of transmittable data will result in a rematch with Luka on this island. She can be rematched the same way as you did in Emerald.
Floreia's Coral Bed (EIP05): A future set's piece of transmittable data will result in a rematch with Floreia on this island. She can be rematched the same way as you did in Emerald.
Magma Base (EIP05): If you have a Pokémon: Game Boy Advance Video Volume 2 game pak in the GBA slot on the DS, you'll now be able to find wild Snorlax inside.
Aqua Base (EIP05): If you have a Pokémon: Game Boy Advance Video Volume 3 game pak in the GBA slot on the DS, you'll now be able to find wild Squirtle inside.
Flora Base (EIP05): If you have a Pokémon: Game Boy Advance Video Volume 1 game pak in the GBA slot on the DS, you'll now be able to find wild Caterpie inside. (If a Japanese Diamond/Pearl is used, the encounters are of Tangela instead.)
Plasma Base (EIP05): If you have a Pokémon: Game Boy Advance Video Volume 3 game pak in the GBA slot on the DS, you'll now be able to find wild Pikachu inside.
Rocket Base (EIP05): If you have a Pokémon: Game Boy Advance Video Volume 1 game pak in the GBA slot on the DS, you'll now be able to find wild Wigglytuff inside.
Cipher Base (EIP05): If you have a Pokémon: Game Boy Advance Video Volume 2 game pak in the GBA slot on the DS, you'll now be able to find wild Misdreavus inside.
Snagem Base (EIP05): If you have a Pokémon: Game Boy Advance Video Volume 4 game pak in the GBA slot on the DS, you'll now be able to find wild Dewgong inside.
Lucy's Pike (EIP05): No changes.
Greta's Arena (EIP05): No changes.
Anabel's Tower (EIP05): The island is bare and the data on the base island doesn't work in Diamond/Pearl. However, while the island is transmitted in, there is a chance that you'll face Anabel in the Diamond/Pearl Battle Tower.
Aeris's Grave (FF7): The previous medium Aeris was communicating through, Rebecca, died just prior to the events of Ranger. Aeris is now communicating through a new medium named Linda in Diamond/Pearl.
Barret Headquarters (FF7): The shop's selection is expanded. The one who gave a shining Minun in the third generation instead gives a shining Plusle in the fourth. If there is a Final Fantasy Tactics Advance game pak in the GBA slot on the DS, there is also a good variety of invisible items all over the ground on the base island.
Cait Sith Jungle (FF7): The Pokémon variety has increased.
Cid Field (FF7): The sky zone here covers most of Sinooh and is again split into zones. Here's how each of the zones falls into place, and which Pokémon appear in which zone:

There are also some wind drifts in the sky zones here. They're one-way passages between an otherwise isolated zone and a zone on the mainland. There's a drift departing from Sky Suicune that leads to Sky Fuego. The return-way drift departs from Sky Eterna-F and leads to Sky Scizor. There's a drift departing from Sky 210 that leads to Sky Fight; its return-way drift departs from Sky Resort and leads to Sky 224.
There is also an exclusive area you can visit if you have a Final Fantasy I & II: Dawn of Souls game pak in the GBA slot on the DS. A drift in the southeast area of Sky Palpark will lead to an area called "Sky Tropics-W". If you descend from here, you'll be in a special area of Cid Tropics that you can't normally access. It's west of the area where Cid takes you in the airship. If you come here with an Articuno, a Zapdos, and a Moltres in your party, you'll open up two shrines in this area, one ascending, the other descending. The descending shrine goes into the "Whirlpool Descent", a confusing underground maze that leads to a Dive spot. Dive down and you'll find a Lugia. You can battle it like any other standing legendary. This Lugia is level 70. The ascending shrine, on the other hand, is a confusing building maze that leads to a roof with a Fly pad. When you use the Fly pad, you'll get caught in a drift that throws you into "Sky Tropics-E". There's an area of land in this sky zone, and on it is a Ho-Oh. You can battle it like any other standing legendary. This Ho-Oh is level 70. Cid and his airship will also be on the scene, asking how you managed to find this place, and will offer you a ride back down to the main area of Cid Tropics; this is how you leave Ho-Oh's area.
Cloud's Reactor (FF7): The number of Enigma Berry is changed in Diamond/Pearl. This berry is still linked to the Sun Stone. Other berries with changed numbers have had their respective stones changed. Transmittable berries now have different stones linked to it depending on their tens digit. If a transmittable berry's tens digit is 0, 3, 6, 9, C, or F, you'll get a Shiny Stone. If a transmittable berry's tens digit is 1, 4, 7, A, or D, you'll get a Dusk Stone. If a transmittable berry's tens digit is 2, 5, 8, B, or E, you'll get a Dawn Stone.
Red XIII's Volcano (FF7): If you have a Final Fantasy IV Advance game pak in the GBA slot of the DS, there is another entrance area you can access. This entrance area allows you to scale the volcano's exterior, where Rock Climb is pretty much a necessity. This area has quite a few wild Numel and Slugma. At the top of the volcano is a pair of binoculars; you can use these to gaze at Trainers' Resort from here.

In addition, if you have a Final Fantasy V Advance game pak in the GBA slot on the DS, once you've beaten Sephiroth in the maze, you'll now have an option in the sky zone's Pokémon Center to take on a brand-new maze. It's largely a simple maze with randomized trainers brandishing random Dark-type Pokémon. However, these trainers have the Final Fantasy V Advance playable classes. There are also wild Skuntank roaming around in the maze, and they're too abundant for Repel to keep away. There's no quiz machine at the end, however. The maze will switch back to maze 9 if you return without a Final Fantasy V Advance game pak in the GBA slot after having enabled the bonus maze.
Tifa Highway (FF7): The highway now wraps around the entire complex. For details on how the road wraps around, look at the transmittable Sonic Riders track "Sinooh Resort Highway"--the base island is the fog-covered island from that transmittable track. You can now choose to go westbound or eastbound at anytime. Additionally, the traffic depends on the clock's hour hand. Traffic is at its highest in the 9:00 AM and 5:00 PM hours, and is at its lowest in the 1:00 AM hour.
Vincent City (FF7): The base island is now covered in fog, as is the Vincent City Cavern. You now need both Flash and Defog to see the Fly tile and to make it through the cavern.
Yuffie's Dojo (FF7): If there is a Final Fantasy VI game pak in the GBA slot on the DS, once you've cleared Yuffie's normal challenge, you'll now have an option to take on a level 2 challenge whenever you reach Yuffie's floor. (For the record, you can still take the level 1 challenge even if you have the option to take level 2.) Level 2 has triple the doors and a four-minute time limit. If you manage to clear level 2, Yuffie will give you a TM60. You can keep winning more TM60s by beating level 2 over and over. Level 2 is only available if you have a Final Fantasy VI game pak in the GBA slot on the DS.
Draco Base (EIP06): If you have a Pokémon: Game Boy Advance Video Volume 4 game pak in the GBA slot on the DS, you'll now be able to find wild Onix inside.

Harley's Funhouse (EIP06): A room that was empty in the third generation now has a chance of a wild Chimchar ambushing you upon entering the room in the fourth. The highest chance of it happening is every 10 room encounters.
Robert's Jet Set (EIP06): For every 10th step you take inside the building, there is now a very small chance of encountering a wild Piplup. Only actual steps will work; Sweet Scent doesn't increase your odds.

Take it to Prof. Oak at the Research Lab and he'll tell you that it's the egg of a Manaphy. If you have an open spot in your party, the egg will leave your key item inventory and become a normal hatchable egg. You can only get one Special Egg in this way.

The slot machines pay out more upon landing good combinations if you have a Sonic X: Game Boy Advance Video Volume 2 game pak in the GBA slot on your DS.
Icecap (EIP06): If you have a Sonic Battle game pak in the GBA slot on the DS and complete the snowboard run in 2 minutes 30 seconds or less, Shadow will appear at the finish line and give you a TM79.
Twinkle Park (EIP06): In the bumper car game, X is now the slide button and Y is now the data-toggle button. In the mirror room, if you have a Sonic Pinball Party or Pokémon Pinball: Ruby and Sapphire game pak in the GBA slot on the DS, some Game Corner coins have blended in with the floor, and you can pick them up like any other invisible item.
Speed Highway (EIP06): If you have a Sonic Advance 2 game pak in the GBA slot on the DS and manage to complete the highway trial within 105 seconds, Sonic will appear at the finish line and give you a TM91.
Red Mountain (EIP06): Gulpin can now appear on the inside of the volcano. If you have a Sonic Advance 3 game pak in the GBA slot on the DS, you'll find Knuckles somewhere on the outside of the volcano. He'll give you a TM61 if you talk to him.
Sky Deck (EIP06): You'll now find Magnezone on the inside of the ship, but this is a very rare encounter. If you have a Sonic the Hedgehog Genesis game pak in the GBA slot on the DS, you'll find Eggman somewhere aboard. Talk to him and you'll get a TM52.
Lost World (EIP06): The center cave formation is now taller and has entrances on three levels. The Jungle Cave is still the topmost cave but its only floor is now 3F (it still leads to 2F of Boulder Cave), and the Boulder Cave is still right below it. There's a new cave on the bottom of the formation called the "Iron Cave". There's only one floor here (1F). There are Steel-type items here, with wild Cranidos and Shieldon abundant, with rare appearances of Rampardos and Bastiodon. The end of that path also puts you on 2F of Boulder Cave. Roark is the one that gives you the prize package in Diamond/Pearl, and the prize package now also includes 5 TM69s, 5 TM71s, 5 TM76s, and 5 TM80s in addition to what was there before.
Final Egg (EIP06): The first area of the maze now has wild Roserade, Chimecho, Sudowoodo, and Blissey. The second area of the maze now has wild Mr. Mime, Snorlax, and Mantine, and the encounters with Electabuzz and Magmar are replaced with encounters with Electivire and Magmortar, respectively.
Hot Shelter (EIP06): The selection of fish have expanded in all three areas.
Tiny Chao Garden (DE): The password that transmits this island doesn't work for Diamond/Pearl, meaning that this island is incompatible with Diamond/Pearl. The island is incompatible due to the way the Tiny Chao Garden is programmed. (A different piece of data from a future set will enable a different Chao-raising feature in Diamond/Pearl. You know the Pokétch's just begging for one...)
For this piece of data, you'll need the password on the "Rowan of Sinooh" nonplayable insert included in this set's booster packs to the Tajiri server, using the universal confirmation password "C4F-4M0-Vxx", where xx denotes a two-letter version identifier; here, DI is for Diamond and PE is for Pearl. Once the transmission's done, you'll recieve a new island that only works in the fourth generation (you'll get an error message if you try transmitting it to Ruby, Sapphire, FireRed, LeafGreen, or Emerald). Here's the details of it:
"Rowan's Remote Lab" (transmit Rowan of Sinooh): Nothing appears in the grasses outside the lab at first. You'll find Prof. Rowan inside the laboratory. Talk to him and he'll check features 10 and 20 of the Pokétch. After he's done so, the non-legendary Pokémon shown in those features will be the ones that will appear in the grasses outside. For example, if feature 10 showed Turtwig, Chimchar, Piplup, Roserade, Buizel, Chatot, Mantyke, Bonsly, Mime Jr., Munchlax, Weavile, and Lucario, those 12 will appear in the grasses. They'll be joined by those shown in feature 20--for example, if feature 20 showed Buizel, Pichu, and Azurill, Buizel will have a higher encounter rate using the previous feature 10 example, while Pichu and Azurill will also appear in the grasses. You can change the Pokémon setup by talking to Prof. Rowan again.
The music heard outside on this island is the Sandgem Town theme from Diamond/Pearl. The music heard inside the lab is the music that plays inside Prof. Rowan's lab in Diamond/Pearl.

The western one is eight 8-square-high columns and, when you first arrive, contains each of the berries found in the Diamond/Pearl in-game (there's 64 total). The eastern one is fifteen 10-square-high columns and one 9-square-high one (with the sign below the 9-square-high one) and, when you first arrive, contains each of the berries obtained through DS transmission that you've unlocked so far (the potential ceiling for this is 159, although there are doubts that this will be reached). There are no other changes.
"Ten" Island (EIP05/SPS2): No changes.
Sonic's Island (SAE/SPS2): The Pokémon variety has increased.
The e-Reader-transmitted version of the GBA island is the one whose music is used.
Tails's Island (SAE/SPS2): The Pokémon variety has increased. The sky zones on this island cover the entire complex that includes Enigma Island (that is, Sky Enigma, Sky Ludicolo, Sky Trainers', Sky Suicune, and Sky Scizor). Rayquaza may appear in the Diamond/Pearl sky zones, but you'll need to have an Emerald game pak in the GBA slot on your DS, and you can only find a Rayquaza in the sky zones while Tails's Island is the active Enigma Island. Additionally, you can only catch one Rayquaza in this location in Diamond/Pearl and Rayquaza is at level 70 (as opposed to 10 in the non-Emerald GBA sky zones).

Eggman's Resort (SAE/SPS2): Budew, Chingling, Bonsly, Mime Jr., Happiny, and Munchlax join the variety found in the grasses. You can now find Mantyke by surfing. All of those Pokémon can now be found in the eggs with the same moves as previously, except if you get a Mantyke from the right-hand egg, it will know Dragonbreath instead of Surf.

Chao Paradise (SAE/SPS2): The Chao transfer method is different in Diamond/Pearl. There are now two of them. The first is to insert up a Sonic Advance, Sonic Advance 2, or Sonic Pinball Party game pak in the GBA slot on the DS. You can transfer the Chao to that game's Tiny Chao Garden in that way. The other is to load up Sonic Adventure 2 Battle or Sonic Adventure DX: Director's Cut into a Wii, upload the password transmission program used for transmitting data to the Wii, and then make a transfer into a Chao Garden for the matching game on any Gamecube memory card inserted into the matching slots on the Wii.

Giovanni's Lab (EIP04): You can choose between a Deepseascale and Deepseatooth when you come here for the first time. Tate & Liza's Skypad (EIP05): Various media reporters, be they TV, radio, newspaper, web site, etc., are now swarming the base island and each of the towers. The ones on the base island will remain there until you reunite Tate & Liza, while the ones in the towers will remain until the towers collapse. You don't have to fight any of the reporters. Tate & Liza have a different team for the rematch.
Cissy Downs (EIP05): The Emerald modifications are in place, complete with the rematch. The actual Surf race now takes place in the same perspective as watching the race in Emerald, and the controls are slightly changed; the GBA L and R are the DS Y and X, respectively. Cissy has a different team for the rematch. Once you've beaten Cissy once with a Sapphire game pak in the GBA slot on the DS, a tsunami will hit the island and the track will be evacuated. A cutscene depicting the track being damaged and the island being flooded will occur, and a dive spot will appear in front of where the door was. Once you use Dive, you'll find a cave entrance. This cave is the Marine Cave, and it works the same as it did in Emerald, right down to obtaining Kyogre. You must catch Kyogre in order to restore the island to normal (if you beat Kyogre without catching it, the Marine Cave won't vanish, the island will remain flooded, and you'll be able to rematch Kyogre on your next visit).
Hotel Luana (EIP05): The Emerald modifications are in place, complete with the rematch. This time around, however, there are now 20 floors to the hotel (there were 15 in the GBA games, something that was never quoted, especially since the 14th and 15th floors weren't important), your hotel room in Diamond/Pearl is 2006 instead of 1337, there are now 19 charms instead of 12, and there are a few slot machines on the ground floor. The new charms can be found in the following rooms: 1337, 1414, 1522, 1606, 1711, 1840, and 1928 (this room is the presidential suite). Luana has a different team for the rematch. Once you've beaten Luana once with a Ruby game pak in the GBA slot on the DS, an earthquake will hit the island and the hotel will be evacuated. A cutscene depicting the hotel being damaged will occur, and a cave will rise in front of the door. This cave is the Terra Cave, and it works the same as it did in Emerald, right down to obtaining Groudon. You must catch Groudon in order to restore the island to normal (if you beat Groudon without catching it, the Terra Cave won't vanish, the hotel will remain damaged, and you'll be able to rematch Groudon on your next visit).
Resort Lorelei (EIP05): Two of the trades are different. The one who has Cloyster now wants a Snover. The one who has Piloswine now wants Sneasel.
Agatha's Dusk Property (EIP05): A Dusknoir in your party has the same effect as Duskull or Dusclops. A Chingling in your party has the same effect as Chimecho. The Emerald alterations to the Duskull-line maze are in place.
Karen Village (EIP05): No changes.
Phoebe's Luau (EIP05): The method for obtaining the invitation is different. To obtain an invitation in Diamond/Pearl, either get one from a held Pokémon in Pal Park or, the first time you transmit this island into Diamond/Pearl, an Event Challenge will open in which you must beat Cal's team in one battle, a single battle with Arceus banned. Win that battle and you'll obtain the invitation. Here's Cal's team for the Event Battle:

Sonrisa Greenhouse (EIP05): Turtwig can now appear in the greenhouse. A future set's piece of transmittable data will result in a rematch with Sonrisa on this island. She can be rematched the same way as you did in Emerald.
Trixie's Space Stage (EIP05): You'll now find Trixie on Ludicolo Lane. The floor of the space station can now wield Piplup. The second warp tile leads to an otherwise-unreachable point in the Scizor Mountains. When Trixie enables the second warp tile, she'll also activate the teleporter that leads to the rest of the Space Station, which is now manned by only two astronauts on the roof in Diamond/Pearl (neither of which want to battle), and all the inside areas now can yield wild Pokémon (only the SE Wing did so in the third generation). A future set's piece of transmittable data will result in a rematch with Trixie on this island. She can be rematched the same way as you did in Emerald.

If there is a Final Fantasy Tactics Advance game pak in the GBA slot on the DS, there is also a good variety of invisible items all over the ground on the base island.

There are also some wind drifts in the sky zones here. They're one-way passages between an otherwise isolated zone and a zone on the mainland. There's a drift departing from Sky Suicune that leads to Sky Fuego. The return-way drift departs from Sky Eterna-F and leads to Sky Scizor. There's a drift departing from Sky 210 that leads to Sky Fight; its return-way drift departs from Sky Resort and leads to Sky 224.
There is also an exclusive area you can visit if you have a Final Fantasy I & II: Dawn of Souls game pak in the GBA slot on the DS. A drift in the southeast area of Sky Palpark will lead to an area called "Sky Tropics-W". If you descend from here, you'll be in a special area of Cid Tropics that you can't normally access. It's west of the area where Cid takes you in the airship. If you come here with an Articuno, a Zapdos, and a Moltres in your party, you'll open up two shrines in this area, one ascending, the other descending. The descending shrine goes into the "Whirlpool Descent", a confusing underground maze that leads to a Dive spot. Dive down and you'll find a Lugia. You can battle it like any other standing legendary. This Lugia is level 70. The ascending shrine, on the other hand, is a confusing building maze that leads to a roof with a Fly pad. When you use the Fly pad, you'll get caught in a drift that throws you into "Sky Tropics-E". There's an area of land in this sky zone, and on it is a Ho-Oh. You can battle it like any other standing legendary. This Ho-Oh is level 70. Cid and his airship will also be on the scene, asking how you managed to find this place, and will offer you a ride back down to the main area of Cid Tropics; this is how you leave Ho-Oh's area.
Cloud's Reactor (FF7): The number of Enigma Berry is changed in Diamond/Pearl. This berry is still linked to the Sun Stone. Other berries with changed numbers have had their respective stones changed. Transmittable berries now have different stones linked to it depending on their tens digit. If a transmittable berry's tens digit is 0, 3, 6, 9, C, or F, you'll get a Shiny Stone. If a transmittable berry's tens digit is 1, 4, 7, A, or D, you'll get a Dusk Stone. If a transmittable berry's tens digit is 2, 5, 8, B, or E, you'll get a Dawn Stone.
Red XIII's Volcano (FF7): If you have a Final Fantasy IV Advance game pak in the GBA slot of the DS, there is another entrance area you can access. This entrance area allows you to scale the volcano's exterior, where Rock Climb is pretty much a necessity. This area has quite a few wild Numel and Slugma. At the top of the volcano is a pair of binoculars; you can use these to gaze at Trainers' Resort from here.

In addition, if you have a Final Fantasy V Advance game pak in the GBA slot on the DS, once you've beaten Sephiroth in the maze, you'll now have an option in the sky zone's Pokémon Center to take on a brand-new maze. It's largely a simple maze with randomized trainers brandishing random Dark-type Pokémon. However, these trainers have the Final Fantasy V Advance playable classes. There are also wild Skuntank roaming around in the maze, and they're too abundant for Repel to keep away. There's no quiz machine at the end, however. The maze will switch back to maze 9 if you return without a Final Fantasy V Advance game pak in the GBA slot after having enabled the bonus maze.
Tifa Highway (FF7): The highway now wraps around the entire complex. For details on how the road wraps around, look at the transmittable Sonic Riders track "Sinooh Resort Highway"--the base island is the fog-covered island from that transmittable track. You can now choose to go westbound or eastbound at anytime. Additionally, the traffic depends on the clock's hour hand. Traffic is at its highest in the 9:00 AM and 5:00 PM hours, and is at its lowest in the 1:00 AM hour.
Vincent City (FF7): The base island is now covered in fog, as is the Vincent City Cavern. You now need both Flash and Defog to see the Fly tile and to make it through the cavern.
Yuffie's Dojo (FF7): If there is a Final Fantasy VI game pak in the GBA slot on the DS, once you've cleared Yuffie's normal challenge, you'll now have an option to take on a level 2 challenge whenever you reach Yuffie's floor. (For the record, you can still take the level 1 challenge even if you have the option to take level 2.) Level 2 has triple the doors and a four-minute time limit. If you manage to clear level 2, Yuffie will give you a TM60. You can keep winning more TM60s by beating level 2 over and over. Level 2 is only available if you have a Final Fantasy VI game pak in the GBA slot on the DS.
Draco Base (EIP06): If you have a Pokémon: Game Boy Advance Video Volume 4 game pak in the GBA slot on the DS, you'll now be able to find wild Onix inside.
Casey Diamond (EIP06): The selection at the shops are expanded, as have the item availability in the locker rooms. The team for the 5th matchup has changed, here's the lowdown:

Harley's Funhouse (EIP06): A room that was empty in the third generation now has a chance of a wild Chimchar ambushing you upon entering the room in the fourth. The highest chance of it happening is every 10 room encounters.
Robert's Jet Set (EIP06): For every 10th step you take inside the building, there is now a very small chance of encountering a wild Piplup. Only actual steps will work; Sweet Scent doesn't increase your odds.

Take it to Prof. Oak at the Research Lab and he'll tell you that it's the egg of a Manaphy. If you have an open spot in your party, the egg will leave your key item inventory and become a normal hatchable egg. You can only get one Special Egg in this way.

Chill Penguin's Getaway (EIP06): If you have a Mega Man Battle Network 5: Team Protoman game pak in the GBA slot on your DS, somewhere on the island lies an "Ice Emblem" (key item). If you have it, you'll be able to find a L50 Regice roaming around in the grasses. Once you catch it, the Ice Emblem will disappear from your inventory. You can only get one Regice in this way.

Casinopolis (EIP06): The slot machines have better odds of landing good combinations if you have a Sonic X: Game Boy Advance Video Volume 1 game pak in the GBA slot on your DS. The slot machines pay out more upon landing good combinations if you have a Sonic X: Game Boy Advance Video Volume 2 game pak in the GBA slot on your DS.
Icecap (EIP06): If you have a Sonic Battle game pak in the GBA slot on the DS and complete the snowboard run in 2 minutes 30 seconds or less, Shadow will appear at the finish line and give you a TM79.
Twinkle Park (EIP06): In the bumper car game, X is now the slide button and Y is now the data-toggle button. In the mirror room, if you have a Sonic Pinball Party or Pokémon Pinball: Ruby and Sapphire game pak in the GBA slot on the DS, some Game Corner coins have blended in with the floor, and you can pick them up like any other invisible item.
Speed Highway (EIP06): If you have a Sonic Advance 2 game pak in the GBA slot on the DS and manage to complete the highway trial within 105 seconds, Sonic will appear at the finish line and give you a TM91.
Red Mountain (EIP06): Gulpin can now appear on the inside of the volcano. If you have a Sonic Advance 3 game pak in the GBA slot on the DS, you'll find Knuckles somewhere on the outside of the volcano. He'll give you a TM61 if you talk to him.
Sky Deck (EIP06): You'll now find Magnezone on the inside of the ship, but this is a very rare encounter. If you have a Sonic the Hedgehog Genesis game pak in the GBA slot on the DS, you'll find Eggman somewhere aboard. Talk to him and you'll get a TM52.
Lost World (EIP06): The center cave formation is now taller and has entrances on three levels. The Jungle Cave is still the topmost cave but its only floor is now 3F (it still leads to 2F of Boulder Cave), and the Boulder Cave is still right below it. There's a new cave on the bottom of the formation called the "Iron Cave". There's only one floor here (1F). There are Steel-type items here, with wild Cranidos and Shieldon abundant, with rare appearances of Rampardos and Bastiodon. The end of that path also puts you on 2F of Boulder Cave. Roark is the one that gives you the prize package in Diamond/Pearl, and the prize package now also includes 5 TM69s, 5 TM71s, 5 TM76s, and 5 TM80s in addition to what was there before.
Final Egg (EIP06): The first area of the maze now has wild Roserade, Chimecho, Sudowoodo, and Blissey. The second area of the maze now has wild Mr. Mime, Snorlax, and Mantine, and the encounters with Electabuzz and Magmar are replaced with encounters with Electivire and Magmortar, respectively.
Hot Shelter (EIP06): The selection of fish have expanded in all three areas.
Tiny Chao Garden (DE): The password that transmits this island doesn't work for Diamond/Pearl, meaning that this island is incompatible with Diamond/Pearl. The island is incompatible due to the way the Tiny Chao Garden is programmed. (A different piece of data from a future set will enable a different Chao-raising feature in Diamond/Pearl. You know the Pokétch's just begging for one...)
For this piece of data, you'll need the password on the "Rowan of Sinooh" nonplayable insert included in this set's booster packs to the Tajiri server, using the universal confirmation password "C4F-4M0-Vxx", where xx denotes a two-letter version identifier; here, DI is for Diamond and PE is for Pearl. Once the transmission's done, you'll recieve a new island that only works in the fourth generation (you'll get an error message if you try transmitting it to Ruby, Sapphire, FireRed, LeafGreen, or Emerald). Here's the details of it:
"Rowan's Remote Lab" (transmit Rowan of Sinooh): Nothing appears in the grasses outside the lab at first. You'll find Prof. Rowan inside the laboratory. Talk to him and he'll check features 10 and 20 of the Pokétch. After he's done so, the non-legendary Pokémon shown in those features will be the ones that will appear in the grasses outside. For example, if feature 10 showed Turtwig, Chimchar, Piplup, Roserade, Buizel, Chatot, Mantyke, Bonsly, Mime Jr., Munchlax, Weavile, and Lucario, those 12 will appear in the grasses. They'll be joined by those shown in feature 20--for example, if feature 20 showed Buizel, Pichu, and Azurill, Buizel will have a higher encounter rate using the previous feature 10 example, while Pichu and Azurill will also appear in the grasses. You can change the Pokémon setup by talking to Prof. Rowan again.
The music heard outside on this island is the Sandgem Town theme from Diamond/Pearl. The music heard inside the lab is the music that plays inside Prof. Rowan's lab in Diamond/Pearl.
