How are swiss rounds and topcut numbers determined?

I know that they both depend on the number of players you have at your tournament but I was wondering how many players dictate how many rounds of swiss you play and how many people will make top cut. If those numbers vary from tournament to tournament I am curious as to how this works for regionals. Thanks!
 
This information is from the Tournament Operations Procedures document found in the Rules and Resources area on the My Pokemon Page:

Players per Age Division (Pod) / Rounds:
6-8 / 3
9–16 / 4
17–32 / 5
33–64 / 6
65–128 / 7
129–256 / 8

This is for Swiss and Top Cut formats.

All Premier events have a maximum Top Cut, dictated to Organizers by TPCi.
 
The maximum Top Cut for regionals, as Wayne says, is set by TPCi - in this case top [del]16[/del], er, 32.

Organizers are allowed to 'play one swiss round' if the venue time is constrained; in the table above the 33-64 would become a 5 round swiss. Typically, the venues for States and Regionals are required to be such that this option is never used.
