Once during your turn (before your attack), if Unown G is on your Bench, you may discard all cards attached to Unown G and attach Unown G as a Pokémon Tool card. As long as Unown G is attached to a Pokémon, prevent all effects of attacks, excluding damage, done to that Pokémon.

Is a Special Condition removed, when I attach Unown to a Pokemon that is already under SC ?

And by Cleffa I mean the HGSS one.
Only damage is prevented, so Cleffa may be poisoned, and it doesn't take any damage as long as it sleeps. So, when it wakes up, I must put damage counters just as on regular poisoned Pokemon, right?

No for two reasons.
First, it only prevents effects from attacks.
If an effect is already there, or a Special Condition, which is a whole category of their own, those would remain.

Second, Unown G has an errata. It only prevents effects from an opponent's attack. Not from your own attacks.

Things that do damage and placing damage counters are two different things.

When Cleffa is Asleep, an attack can't do damage to it.
That doesn't prevent an attack from placing damage counters or damage counters being placed by Special Conditions or other effects such as Flash Bite.
