Lets go straight to the scenario:

Player 1 has 1 card in hand and it lets him draw UP TO 6 cards in hand. Player 1 plays the card and starts to draw. Player 1 takes 1 cards at a time and stops at 4 telling his opponent he would like to stop here. Player 2 says player 1 should have told him how many cards he wanted to draw before he played the card. Player 2 calls a judge. How do you handle this situation. Also what are the penalties (if any) for both players in this scenario since both didn't pay attention to the game state at 1 point
 
Are you asking about a specific effect? If so, best to name it because the exact wording is important in ALL rulings.
 
It was actually something which just popped up in a discussion, but a card like Giratina from Legends awakening (9/127) could be used as a reference. Player 2 calls out he draws 4 cards. Player 1 stops at 3 without saying how many cards he wanted to draw.
 
If a player doesn't declare that they are not drawing the maximum, the default is to draw the maximum.
This can start as Game Play Error Minor with a Caution, but it can escalate if the player continues to purposely ignore the correct way to do it.
