I think Clue # 2 means a dark, poison, bug, ghost type or any Pokemon looked really ugly, but appreantly not.
It might be friendly evolution for Heart, but appreantly not. The only ones left are Cleffa, Azurill and Togepi.
Mysterious... any pokemon is mysterious, is it? Even fossils.
Catterpiller is not caterpillar. I had no clues either.
Twice. Evolve twice? Twice as...?

She actually showed up 3 times, once with a Chikarita, then in the 2nd it evolved to Bayleaf, Ash gave her a Beedrill, then in a 3rd it was already a Meganium, she caught an Elekid. Also, I believe it was already said.

I will guess Tangela, it is in mysterious vines, it makes gooey grass attacks, it has vines, that are worm-caterpillar like, it was only in two sets of pokemon games (r/b/y, g/s/c), and treecko has a special place in his/her heart for this special grass type.
