KO-ing Pokes/Switching

I've got something I need clarification on:

If I have Froslass active and KO the Defending Pokemon with Icy Breath, does it place the damage counter on the pokemon immediately, or is it after my opponent brings up his new poke, thus the new poke not taking the damage counter? I know it doesn't say "then" on the card, buy I'm also pretty sure that a player never doesn't have an active pokemon (double negative intended); The KO-ing and the bringing up of the new active pokemon happen simultaneously, though I'm not sure.

The Bench damage is immediate. So whomever becomes the new Active gets the damage put on it before becoming the new Active

More generally, all effects of an attack are resolved before removing knocked-out Pokemon from play.
