While a player is setting up, if a prize card is accidentally revealed, does that card get shuffled back into the deck? Or does it remain a prize?
 
That's not the kind of question that we can answer in the ATRT forum.
That is a judge call and is better asked about in the Judge's Chambers forum.

GPE Minor- Caution at Tier 1, Warning at Tier 2

Leave it as a prize, randomize the prizes. Tournaments are competitive events, most players know what is in their prize cards after the 1st/2nd search anyways.
 
Since the deck is considered to be random in terms of both content and order the error can be completely unwound. There is no reason why either player should be advantaged or disadvantaged by an accidentally revealed prize card during set up. This would not be the case if the deck was considered to be random but possessing a yet to be revealed order.

Along with the caution/warning, I'd return the revealed card to the deck, shuffle it, and then continue to place the prizes.
